The Primary Health Network Charitable Foundation is the sole fundraising arm of the Primary Health Network. PHNCF sustains three programs for PHN patients and community members, including the free "Provide the Ride" transportation program and the "Caring for Our Neighbors" prescription assistance program, as well as the scholarship program. Also established in 2005, PHNCF works to support the mission of the Primary Health Network to increase access to healthcare services that improve the quality of life for PHN patients.
