Illinois Health Connect is a healthcare program of the Illinois Department of Healthcare and Family Services (HFS). Most people with an HFS or All Kids Medical Card MUST pick a Primary Care Provider (PCP) for their medical home. With a medical home, you and your family get quality healthcare to stay healthy. When you enroll in Illinois Health Connect, you will have: A "medical home" for all of your health care needs. A Primary Care Provider (PCP) who gets to know you well. I am a project manager with a focus on creating systems and protocols that are sustainable over time. Some of my proudest accomplishments include the overhaul of the IL Angel Investment Tax Credit Program that reduced turnaround time on applications for approval by 70%, the strategic plan in the Office of Entrepreneurship, Innovation and Technology, where we launched 70 individual projects focused on sustainability of our programs and the streamlining of Empire Zones Program applications in NYC to eliminate confusion by our city vendors.
